Yes, it's generally the sign of a mechanical failure (damaged read heads). Switch off the drive immediately and bring it to me without switching it on again. Each extra boot cycle degrades the data. I never open a drive: a physical failure goes to a partner lab equipped with a clean room, by quote.

Often yes, on a conventional hard drive: as long as you haven't written new data since, the files are recoverable. Be warned, though: on a recent SSD, deleted files are often erased permanently within a few minutes (the TRIM function), and the diagnosis tells you so honestly. Don't use the PC in the meantime.
